Support plan
When we agree to provide support services to a customer one of the first things we do is draw up a support plan.
What does the plan do?
A support plan assists our service users to identify achievable goals and aspirations.
The plan sets out practical assistance, guidance, encouragement and training designed to help customers reach their goals and develop independence.
How is the plan made?
Each plan is agreed with the service user and is individualised and flexible and can be updated at any time.
It is reviewed at least every six months by a support worker. The review will include the service users assessment of progress towards achieving their goals.
A support plan can include input from other statutory and voluntary services for example, social services, the Citizens Advice Bureau, housing, employment and training, faith, sport and recreational services.
